An eyelid lift or blepharoplasty, involves removing bulging fat
and skin from the upper and lower eyelids, and results in giving
the eyelids a more rejuvenated and fresh appearance. An eyelid
lift will reduce the saggy appearance of the upper lids, and a
lower blepharoplasty will help the puffy, tired look of the lower
lids.
Who is a candidate for eyelid or brow surgery?
If you have excess skin that is weighing down your upper eyelids
and blocking your peripheral (outside) vision, you may need corrective
surgery for medical reasons. The excess skin of the upper lid
may actually be from the forehead skin drooping down, and you
will be evaluated to determine what is the cause of the excess
skin. Some people require a brow lift rather than upper eyelid
surgery to correct the problem, and sometimes it is a combination
of forehead skin and excess eyelid skin. If you appear to have
excess skin blocking your peripheral vision, visual field testing
will be done to check this, and if appropriate, the procedure
will be preauthorized with your insurance.
Eyelid and brow surgery can also be performed for cosmetic reasons,
to help alleviate the heaviness around the eyes and give them
a fresher, younger appearance. Women especially will tend to look
stern or angry as their brows descend, and lifting them can often
lighten their appearance, giving them a friendlier, more open
countenance.
How is an eyelid lift performed?
For the upper eyelid, an incision is made in the eyelid crease,
extending out to the side of the eye. Excess skin is removed,
and sometimes a little fat is also taken, if there is bulging
along with the excess skin. Tapes are put over the incision, and
stitches and tapes are removed in 3-4 days, since eyelid skin
is very thin and can heal quickly. This procedure is frequently
done in the office under local anesthesia.
For the lower lid, the incision is just below the lashes, extending
out to the side. Fat is removed if there is puffiness, and excess
skin is removed. Often the lower lid itself is also tightened
in the outside corner. Tapes are placed under the eye and stitches
and tapes are removed 3-4 days after surgery. This surgery is
usually done in the office or ambulatory surgery center.
What can be expected after eyelid surgery?
Tapes are placed on the eyelid and are usually removed with the
stitches in 3-4 days. Some bruising will occur, and can be corrected
with camouflage makeup after the tapes are removed. Bruising will
usually take 3-4 weeks to disappear. Most people can return to
work in 3-4 days depending on their type of work. Vigorous exercise
usually can be initiated within a week or two.
What is a brow lift?
A brow lift is done to elevate the brows and forehead that have
sagged over time. It will result in a fresher, younger appearance
since elevating the brows helps open up the eye area. It also
lightens the area between the brows, tightening the skin and reducing
wrinkles.
The placement of the incision for the brow lift depends on the
height of the forehead and the presence or absence of hair. The
incisions may be done within the hair, placing four short incisions
and using a scope to perform the surgery, or placed along the
edge of the hairline if the forehead is already high. Men that
are balding may require the incision to be placed in a wrinkle
in the middle of the forehead. The surgery is usually performed
as an outpatient surgery through an accredited ambulatory surgery
center.
What can be expected after a brow lift?
Brow lift surgery is usually an outpatient procedure. The patient
will be sent home with a dressing wrapped around the head (the
hair is not shaved) which is usually removed in 24-72 hours. The
hair can be washed once the dressing is removed. Staples or stitches
are removed in 5-10 days, depending on what type of brow lift
is performed.
How do I know if I need a brow lift or eyelid surgery?
Dr Wray will examine your face, evaluating what is causing the
problem of the excess skin, evaluating whether there is blockage
of the peripheral vision, and then accessing the areas that will
need to be addressed.
If the brows are down, she will assess the height of the forehead
and the hair pattern to determine the best approach for a brow
lift. Sometimes the problem is both the brows being down and excess
eyelid skin, so at times both a brow lift and eyelid surgery are
indicated.
Can eyelid surgery be redone?
If over time excess eyelid skin recurs , eyelid surgery can be
done again. Sometimes a brow lift is what is needed the second
time around, since taking out too much eyelid skin can pull the
brows down. This would be assessed at the time of your consult.
Is eyelid or brow surgery painful?
Usually upper eyelid surgery is done in the office, and patients
frequently just use Tylenol or ibuprofen for discomfort. Lower
lid surgery can be done in the office or as an outpatient surgery
through an ambulatory surgery center, and patients may require
a prescription pain medication for a day or so. Brow lift surgery
is most often done as an outpatient surgery through an ambulatory
surgery center, and the patient may require prescription pain
medication for a few days.
